A provider who answers all five in writing is worth more than one who is 15% cheaper.

Week one: team assigned, access provisioned, an audit of what you currently have. Week two: a prioritised backlog you agreed to, and the first demo. By day thirty you should have shipped something visible, closed the worst security or performance gap, and have a written roadmap for the next quarter. If a month has passed and you have only had meetings, that is your signal.
